The terms of this consent form outline an agreement between your organization and NCSi, Inc. for immediate technology assistance. The client releases NCSi, Inc. from any liability related to the services, agrees to maintain confidentiality, accepts payment terms, and commits to a consultation meeting for potential IT services.

I, the undersigned, hereby acknowledge and agree to engage NCSi, Inc. to provide immediate technology assistance. This may include, but is not limited to, addressing hardware issues, software troubleshooting, and assessing system configurations.

Release of Liability:

In consideraton of the services to be provided, I, on behalf of myself and my organization, release and discharge NCSi, Inc., its employees, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to the provision of technical assistance and access to our equipment and data.

Confidentiality Agreement:

I understand that during the course of providing technical assistance, NCSi, Inc. may have access to sensitive and confidential information. I hereby agree that NCSi, Inc. will not disclose, share, or use such information for any purpose other than providing the agreed-upon technical assistance.

Payment Agreement:

I agree to pay NCSi, Inc. for the services provided at the rate of $159 per hour for labor, and for any materials used during the service. An invoice will be submitted at a later date, and payment is due upon receipt of the invoice. If I choose to accept a service agreement, NCS may delay the payment request until after the agreement is signed, offering a discounted labor rate.

Consultation Meeting:

I further agree to schedule a meeting with NCSi, Inc. to discuss potential IT services for our organization.

Term and Termination:

This agreement will commence upon the effective date and will continue until the completion of the agreed-upon services. Upon completion of the agreed-upon services, certain provisions of this agreement shall survive, including but not limited to the terms related to release of liability, confidentiality, and payment of services.

By accepting and signing below, I acknowledge that I have read, understood, and agree to the terms and conditions outlined in this Release of Liability and Consent Agreement. The undersigned represents and warrants that they have the authority to enter into this agreement on behalf of my organization.
